1. ACKNOWLEDGEMENT OF RISKS

I understand that participation in Brazilian Jiu-Jitsu, grappling, martial arts training, fitness activities, self-defense instruction, conditioning exercises, competitions, demonstrations, and related activities involves inherent risks.

These risks include, but are not limited to:

• Cuts, bruises, strains, sprains, and muscle injuries

• Broken bones and dislocations

• Concussions and head injuries

• Neck, back, and spinal injuries

• Joint injuries

• Serious permanent disability

• Illness or infection

• Death

I understand that injuries may result from my own actions, the actions of other participants, equipment failure, facility conditions, or the negligence of others.

I voluntarily choose to participate despite these risks.

2. HEALTH DECLARATION

I certify that I am physically capable of participating in training activities and have not been advised by a medical professional to avoid such activities.

I understand that it is my responsibility to consult a physician regarding any medical condition that may affect my participation.

I agree to immediately inform instructors of any injury, illness, or medical condition that may affect my ability to train safely.

3. RELEASE OF LIABILITY

In consideration for being permitted to participate in activities at Mount Dennis Brazilian Jiu-Jitsu Academy, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release and discharge Mount Dennis Brazilian Jiu-Jitsu Academy, its owners, instructors, coaches, volunteers, affiliates, members, assistants, and landlords from any and all liability, claims, demands, actions, or causes of action arising from injury, loss, damage, or death resulting from participation in academy activities, whether caused by negligence or otherwise, to the fullest extent permitted by law.

4. ASSUMPTION OF RESPONSIBILITY

I agree to:

• Follow all academy rules and instructor directions.

• Train responsibly and safely.

• Maintain personal hygiene suitable for close-contact training.

• Wear appropriate training attire.

• Refrain from training while under the influence of drugs or alcohol.

• Refrain from training while knowingly suffering from a contagious illness or skin condition.

I understand that failure to follow these standards may result in suspension or termination of my membership without refund.

5. EMERGENCY MEDICAL TREATMENT

In the event of an injury or medical emergency, I authorize Mount Dennis Brazilian Jiu-Jitsu Academy staff to obtain emergency medical treatment on my behalf if deemed necessary.

I understand that any costs associated with such treatment are my responsibility.

6. PHOTOGRAPHY AND MEDIA RELEASE

I grant Mount Dennis Brazilian Jiu-Jitsu Academy permission to use photographs, video recordings, and other media containing my image for promotional, educational, marketing, and social media purposes without compensation.

I do NOT consent to the use of my image for promotional purposes.

7. PERSONAL PROPERTY

I understand that Mount Dennis Brazilian Jiu-Jitsu Academy is not responsible for lost, stolen, or damaged personal property brought onto academy premises.

8. MEMBERSHIP POLICIES

I acknowledge that:

• Membership fees, billing schedules, and cancellation policies are outlined in my membership agreement.

• Membership fees are subject to the terms agreed upon at enrollment.

• Failure to attend classes does not relieve me of my membership obligations.

• Memberships are non-transferable.

9. MINOR PARTICIPANTS

If signing on behalf of a minor participant, I certify that I am the parent or legal guardian of the participant and agree to all terms of this agreement on behalf of the minor.

I further agree to indemnify and hold harmless Mount Dennis Brazilian Jiu-Jitsu Academy from any claims brought by or on behalf of the minor.

10. GOVERNING LAW

This agreement shall be governed by and interpreted under the laws of the Province of Ontario and the laws of Canada applicable therein.

If any provision of this agreement is determined to be unenforceable, the remaining provisions shall remain in full force and effect.

ACKNOWLEDGEMENT AND SIGNATURE

I acknowledge that I have carefully read this agreement, fully understand its contents, understand that I am giving up certain legal rights, and sign it voluntarily.
